CENTREX Online Applications
We have made the application process as simple as possible by providing the ability to complete forms electronically.
The steps include:

Use for all risks other than retail stores and special events. The submission must include a separate Centrex Application for each location to be insured. For example, if the risk is a 10-location restaurant chain, a separate Centrex Liquor/General Liability Application is required for each location.

Use for Retail Store with no on-premises consumption, such as convenience stores, grocery stores, liquor stores and similar stores with take-out sales of alcohol only. For multi-location store risks, only one Centrex Retail Store Liquor/General Liability Application is required along with a Centrex Multi-Location Supplement.

Use in conjunction with one Centrex Liquor/General Liability Application for insureds with multi-locations and with no on-premises consumption.

Use in conjunction with one Centrex Liquor/General Liability Application for all submissions on risks, other than restaurants, that have catering and/or hall rental operations (i.e., caterers, banquet halls, clubs with hall rental operations, etc.).

Use for special events, such as church festivals, community festivals, arts and craft shows, fund raisers, etc.
